The Social Work Case Manager is responsible for assessing the psychosocial needs of persons living with HIV/AIDS and developing, implementing, monitoring, and evaluating an agreed upon individualized plan of care for each client. Responsible for informing clients about and referring them to appropriate community resources and advocating for services. Through intervention and education, the Social Work Case Manager supports the client in maintaining the highest degree of personal control and independence. Bilingual desired but not required.
