We are currently recruiting for the following position:
 
Nurse Disease Management

Responsible to perform health coaching for members, across all lines, with chronic diseases. Essential duties may include, but are not limited to: Conducts behavioral or clinical assessments to identify individual member knowledge, skills and behavioral needs. Works with the member to identify a specific health coaching plan to address objectives and goals as identified during assessment. Implements care plan by educating members regarding clinical needs and facilitating referrals to health professionals for behavioral learning needs. Implements and/or coordinates coaching and/or care plan. Monitors and evaluates effectiveness of interventions and/or health coaching plans. Modifies health coaching plan as necessary. Interfaces with health professionals to coordinate health coaching plan for the member. Refers member to catastrophic case management as appropriate. Performs other related duties as required.  

Current unrestricted RN license in applicable states and 2-3 years clinical or home health/discharge planning experience, including previous case management experience required. Thorough working understanding of health insurance/benefits industry essential. Previous experience in adult or pediatric, OB/GYN, med/surg, home health, or case management experience preferred. Previous experience with chronic conditions of diabetes, asthma, COPD or cardiac disease experiences and patient education experience preferred. Excellent oral and written communication skills as well as strong organizational and motivational skills required.
