Children’s Hospital of Orange County (CHOC), a 202-bed and growing state-of-the art regional children’s hospital has earned a strong reputation for leadership in pediatric health care across the county & the entire Southern California region. At the Clinic at Para Ninos, we serve the Santa Ana’s community’s children. We are looking for a LVN/Care Coordinator that wants to make a mark.
LVN/CARE COORDINATOR
Costa Mesa clinic
(Full-time, 8 hour shift, Monday through Friday 8am-4:30pm or 8:30am to 5pm)
No weekends. No holidays.
Acts as a patient advocate providing access to treatment programs and specialty care centers through good interpersonal relationships and communication with families, team members, community health care provides/vendors and third party payers. Collaborates with multidisciplinary team to develop the plan of care. Utilizes all community/CHOC resources while problem solving barriers to clinical care and treatment. Identifies all age and culturally appropriate educational and resource needs. Monitors and documents all interventions. Ensures that all patients referred for care management have an established medical home with appropriate funding resources. Ensures all regulatory requirements are met for all governmental programs. Participates in clinical and quality assurance issues utilizing provider and nursing advisors as necessary. Makes referrals/appointments as requested and monitors appropriate utilization of resources for timeliness of intervention and compliance/barriers of families. Implements and maintains a process to track patient plan of care to ensure follow up occurs in a timely manner. Serves as a resource, consultant, communicator and coordinator with other health care workers to establish optimal health care.
Graduate of an accredited LVN school with a current CA LVN license, BLS and PALS. Pediatric experience in an acute care or ambulatory setting. Must be proficient in Microsoft Word and Excel. Bilingual in Spanish preferred. Will consider a new LVN nursing graduate for this position. Strong organizational and customer service skills. Exhibits maturity, self motivation, flexibility and teamwork. Must have a desire to work with children and families in a multicultural setting and be flexible to work at two different clinics.
